Living with lingering symptoms after COVID-19 can be overwhelming. For many people, recovery isn’t a straight line. Lingering or new symptoms can appear weeks, months, or even years after infection, making daily life feel unpredictable and exhausting. Known as Long COVID or Post-Acute Sequelae of SARS-CoV-2 infection (PASC), this condition can affect multiple systems throughout the body — leaving individuals struggling with fatigue, shortness of breath, brain fog, pain, and other complex issues that impact quality of life.
While research is still evolving, studies suggest that ongoing inflammation, immune system changes, tissue damage, and autonomic nervous system dysfunction may contribute to these persistent symptoms. Because every person’s experience is unique, a one-size-fits-all approach rarely works. At Beyond Basics Physical Therapy, we recognize how frustrating and isolating this experience can feel. Our therapists take time to listen, validate your symptoms, and guide you through a gradual, personalized plan designed to help you rebuild strength, function, and confidence through compassionate, one-on-one care.
Common Symptoms and Challenges of Long COVID

Common Symptoms and Challenges of Long COVID

No two people experience Long COVID the same way. Symptoms can fluctuate in intensity and duration, often affecting multiple body systems and making daily activities feel unpredictable. Many patients describe a cycle of progress and setbacks, which can be both physically and emotionally exhausting.

Below are some of the most common symptoms we see among individuals living with Long COVID:

  • Persistent fatigue and post-exertional malaise (PEM) — feeling significantly worse after physical or mental activity
  • Shortness of breath or breathing difficulties that make everyday tasks feel challenging
  • Dizziness, rapid heart rate, or lightheadedness related to autonomic nervous system dysfunction
  • Muscle and joint pain or weakness that limits mobility and endurance
  • Brain fog, headaches, and difficulty concentrating that interfere with work and daily focus
  • Digestive changes such as bloating, constipation, or nausea
  • Sleep disturbances, anxiety, or mood changes that can impact overall well-being
  • Hormonal dysregulation that can impact a number of different systems with mental and/or physical manifestations
  • Pelvic pain, urinary urgency, or sexual dysfunction, often overlooked but commonly linked to Long COVID and chronic muscle tension

At Beyond Basics Physical Therapy, we understand that these symptoms are real, complex, and deeply interconnected. Our therapists look beyond individual complaints to uncover the patterns and systems contributing to your challenges.

If you’ve been searching for answers, take the first step toward compassionate, expert care.

How Physical Therapy Helps with Long COVID

Recovery from Long COVID isn’t about pushing harder — it’s about learning to move forward with care, awareness, and support. At Beyond Basics Physical Therapy in NYC, we meet you exactly where you are. Our therapists focus on rebuilding strength, stability, and confidence in a way that honors your body’s pace and natural healing process.

Together, we’ll create a plan that supports your energy, mobility, and quality of life — one step at a time. Our approach is comprehensive and individualized, addressing movement, function, and the interconnected systems that support your recovery from the inside out.

Orthopedic Physical Therapy

Many people with Long COVID experience muscle and joint pain, stiffness, and deconditioning from illness or inactivity. Our orthopedic physical therapists help you safely regain strength and mobility through gentle, graded exercises, postural and gait training, and manual techniques to release tension and improve circulation. We also address changes in posture and breathing mechanics that may develop from prolonged illness or sedentary recovery, helping you restore proper alignment and efficient movement patterns.

Pelvic Floor Physical Therapy

Persistent coughing, chronic tension, or prolonged inactivity can contribute to pelvic floor dysfunction — leading to symptoms such as pelvic pain, urinary urgency, constipation, or sexual discomfort. Our pelvic health specialists assess and treat these concerns using gentle manual therapy, breathing retraining, and exercises designed to restore coordination, relaxation, and strength of the pelvic floor muscles.

Autonomic & Systemic Support

Long COVID can affect the autonomic nervous system, leading to dizziness, rapid heart rate, or intolerance to standing (symptoms similar to POTS). Our therapists help you understand these changes and work with your body’s signals through breathing regulation, postural reconditioning, and strategies that promote autonomic balance. When appropriate, we coordinate care with your broader medical team to ensure your treatment plan supports every aspect of your recovery.

Education & Empowerment

We believe education is key to long-term healing. You’ll learn pacing techniques to manage post-exertional malaise, how to monitor your energy levels, and ways to integrate gentle movement into your day without triggering setbacks. We also provide customized home programs and self-care strategies to help you stay engaged in your recovery between sessions.

Emotional & Lifestyle Support

Living with Long COVID can take a toll both physically and emotionally. Fatigue, isolation, and uncertainty are common — and you don’t have to face them alone. Our therapists provide encouragement, practical tools, and referrals to trusted mental health professionals and wellness providers when needed. We celebrate progress at your pace, recognizing that healing is not linear but deeply personal. Small, consistent steps lead to lasting change.

After a life changing surgery back in 2013, I dealt with chronic pelvic and lower back pain that had impacted and hindered me for years. At one point, I lost all hope that I would ever be able to be rid of it. Then, I had the fortune of coming across Beyond Basics and meeting a wonderful physical therapist in the form of Kiko. After nearly 13 yrs of dealing with this pain, I am happy to say that it is completely gone after my treatment with Kiko. Because of her and Beyond Basics, I am now able to enjoy life so much more and for that, I am eternally gratefully. Coming here was truly a life changing experience.

I had a great experience working with Emily Moore for pre-op PT. She created an incredibly warm and inclusive environment and helped me feel comfortable from the start. In just a few sessions, she really helped me have a better understanding of my symptoms and how to best manage them. She gave me helpful tips to integrate into my daily routine and I noticed results almost immediately. I am looking forward to resuming sessions with her again after surgical recovery. The front desk and administrative staff were wonderful as well. I highly recommend working with Emily!

I recently saw Kiko for regular PT sessions following a c-section delivery and HIGHLY recommend her, she's the best!! We worked on regaining both pelvic floor and core strength, and while I'm happy both have improved significantly in the months since my delivery, I was sad to have 'graduated' PT as I really enjoyed my sessions with Kiko - she's a pleasure to work with and learn from! I also had a good experience with Beyond Basics as a practice - Veronica at the front desk is very sweet!

Staff is friendly and efficient. Kiko is so easy to talk to, super informative, genuinely caring & kind, very knowledgeable about the pelvic floor as well as how other areas of the body work with it, and she also gave a great referral when my condition seemed to need treatment outside of her scope of practice. I highly recommend Kiko!

Sonali Patel at Beyond Basis is an incredibly smart, kind, and effective pelvic floor PT. She was great during our sessions and also gave me the tools to continue healing on my own, without ever pushing unnecessary visits. I’d absolutely recommend her and will definitely come back to her if anything ever comes up again.
Kiko is exactly what you want in a PT… Attentive and clearly loves what she does, all with a sense of humor. You’ll be lucky if you get her..

Frequently Asked Questions

What is Long COVID?
Long COVID, also known as Post-Acute Sequelae of SARS-CoV-2 infection (PASC), refers to symptoms that continue or appear weeks, months, or even years after a COVID-19 infection. These symptoms can affect multiple systems — including the respiratory, cardiovascular, neurological, hormonal, and musculoskeletal systems — and vary greatly from person to person.
Why am I still tired months after having COVID-19?
Persistent fatigue is one of the most common symptoms of Long COVID. It’s often linked to post-exertional malaise (PEM), where physical or mental activity causes a delayed increase in fatigue or other symptoms. Physical therapy can help by introducing gentle, graded activity and pacing strategies that respect your body’s limits while gradually improving stamina.
Can physical therapy help with Long COVID recovery?
Yes. Physical therapy can play a major role in recovery from Long COVID by improving strength, mobility, breathing, cardiovascular endurance, overall function and reducing pain. At Beyond Basics, our therapists use a personalized approach that focuses on energy conservation, breathing retraining, gentle movement, and symptom management — always progressing at a pace that feels safe for you.
What are the most common Long COVID symptoms?
Common symptoms include fatigue, shortness of breath, dizziness, rapid heart rate, joint or muscle pain, brain fog, sleep changes, hormonal changes, digestive issues, and sometimes pelvic pain or urinary urgency. These symptoms may come and go, and recovery can feel non-linear — that’s why individualized care is essential.
Can Long COVID contribute to pelvic floor problems?
Yes. Chronic coughing, inflammation, and inactivity can all strain the pelvic floor muscles. Some people experience pelvic pain, urinary leakage, constipation, or sexual discomfort as part of their post-COVID symptoms. Our pelvic floor physical therapists specialize in gentle, effective treatments to restore balance and function.
Is it safe to exercise with Long COVID?
Exercise tolerance varies widely from person to person. Pushing too hard can worsen symptoms for some people. That’s why we focus on pacing, postural support, and guided movement rather than intense workouts. Our therapists monitor your body’s response to ensure each session supports recovery, not setback.
How long does it take to recover from Long COVID?
Recovery is highly individual. Some patients notice gradual improvement within a few weeks of consistent therapy, while others may need ongoing support over several months. Our team adjusts your care plan based on your progress, symptoms, and goals.
Can Long COVID affect the nervous system?
Yes. Many individuals experience symptoms related to autonomic nervous system dysfunction, such as dizziness, heart palpitations, or difficulty regulating blood pressure. Our therapists provide breathing and postural retraining to help rebalance your system and improve day-to-day function.
Can Long COVID symptoms come and go?
Yes. It’s common for symptoms to fluctuate in intensity — some days you may feel better, while others feel more challenging. This variability doesn’t mean you’re going backward; it’s simply part of the recovery process. Our therapists help you learn pacing, energy conservation, and movement strategies to navigate these ups and downs safely.
When should I seek professional help for Long COVID symptoms?
If you’re still experiencing fatigue, pain, shortness of breath, or other symptoms that interfere with your daily life weeks or months after infection, it’s time to reach out. The sooner you begin guided rehabilitation, the better your chances for meaningful recovery. Our team can help you get started with an individualized plan tailored to your goals.
